CalAIM Services offered by World Relief

World Relief Sacramento, World Relief Modesto, and World Relief SoCal are now enrolling clients for CalAIM Services!

CalAIM (California Advancing and Innovating Medi-Cal) is transforming Medi-Cal to deliver whole-person care to adults and children—integrating physical health, behavioral health, and social supports. Through Enhanced Care Management (ECM) and Community Supports (CS), we help Medi-Cal members, including refugees and immigrants, achieve stability and better health outcomes. 

Partner with us to connect your clients to the care and resources they need. Together, we can reduce barriers and promote stability, independence, and recovery. 

Why refer? 

Your clients may qualify if they: 

Are Medi-Cal members with complex physical, behavioral, or social needs 

Are experiencing homelessness or housing instability 

Have frequent ER visits or hospitalizations 

   

Face severe mental health challenges 

Services include: 

   

Intensive case management for up to 12 months 

  

Housing transition and tenancy support 

Referrals to health and social services 

  

Support for pregnant and postpartum women
